Título

DISTINCT VISUAL FIELD ASSESSMENT IN THE STRUCTURE-FUNCTION CORRELATION IN EYES WITH BAND ATROPHY OF THE OPTIC NERVE.

Objetivo

To investigate the relationship between macular ganglion cell complex (mGCC) and peripapillary retinal nerve fiber layer (pRNFL) thickness measurements, respectively with central automated perimetry (10-2 strategy with target sizes I, II or III) and the kinetic semi-automatized visual field (VF) in eyes with band atrophy (BA) and healthy controls (CT).

Método

Thirty eyes with BA and permanent temporal VF defect from 25 patients with previous chiasmal compression and 30 eyes from 23 CT were evaluated. Subjects underwent complete ophthalmological examination including standard automated perimetry (24-2 threshold strategy with size III target) at study entry to document a stable temporal VF defect (BA) or normality (CT). Subjects were submitted to 10-2 VF examination with sizes I, II and III target sizes and to kinetic semi-automatized VF examination. Optical coherence tomography (OCT) was used to assess the mGCC and pRNFL thickness measurements. Statistical analysis included group comparisons (Generalized Estimation Equations), and correlations between measurements (Spearman´s test). Areas under the Receiving Operator Characteristic curves (AUC) were calculated and compared. Significance was set at p ≤ 5%.

Resultado

Central 10-2 VF with size I target didn´t achieved statistical significance between BA and CT groups in the nasal hemifield. Spearman’s correlations coefficients rho calculated between 10-2 VF sizes I, II visual loss in the temporal hemifield and the GCCm analysis of the nasal hemiretina was very strong in eyes with BA. Size III target had the best performing AUC in all parameters compared. No significant difference was found when II and III measurements was performed. As for the kinetic VF, the I/3e isopter revealed the best performing AUCs.

Conclusão

Our findings suggests that both central VF and mGCC evaluation are important in compressive neuropathies. Using stimulus of different sizes should be considered while assessing different areas of the VF by respecting the spatial summation and the density of the ganglion cells.
